Breast Augmentation
Breast augmentation surgery, also known as augmentation mammaplasty, involves the use of breast implants to help enlarge the breasts or to restore volume after pregnancy or weight reduction. The goal of this procedure is to create a more natural look by improving the shape, balance and proportion of the breast in relation to the patient's body frame. Dr. Bidic will help create a larger, more youthful appearance to the breasts, while ensuring that they move naturally and feel soft to the touch.
Breast Lift (Mastopexy)
Breast lift augmentation is a very popular procedure for women and among the most commonly performed cosmetic surgery procedures performed throughout the world. Poor skin elasticity, gravity and other factors such as pregnancy, breast feeding and weight loss can lead to a reduction in breast firmness and shape. A breast lift, or mastopexy, corrects these unwanted changes, giving the breast a more youthful appearance and texture. In addition, an augmentation can be performed at the same time to improve shape and volume.
Breast Reduction
A breast reduction procedure is designed to relieve the physical discomfort associated with large breasts while improving overall breast appearance. Women with extremely large or sagging breasts can be both emotionally and physically uncomfortable. They may suffer from neck and back pain, irritation under the breasts, and may also be self-conscious about their breast size. The breast reduction procedure reduces the size and reshapes the contour of the large breasts by removing excess breast skin and breast tissue. Elevating the breast and reducing nipple size is an important part of this procedure.
SIEA Flaps (superficial inferior epigastric artery flaps)
The SIEA flaps reconstruction procedure uses the body's own dispensable abdominal skin and fat tissue to reconstruct a breast after a mastectomy. During this process no dissection of the abdominal muscle is required, and the use of the superficial vessels in SIEA allows for collection of this tissue without sacrifice of underlying abdominal muscles. After this procedure, restoration of the nipple and areola are performed. The inevitable scarring will substantially fade over time. In many cases, women have felt that their new reconstructed breast is actually firmer and more youthful in appearance that their natural breast. As an added benefit, patients who undergo the SIEA procedure also receive a bonus abdominoplasty (tummy tuck) at the same time. Dr. Bidic has extensive experience with the SIEA flap procedure and has been performing this procedure throughout his career.
TRAM Flaps (transverse rectus abdominus myocutaneous flaps)
The TRAM flap is a tissue flap procedure that uses muscle, skin and fat from a patient's abdomen to create a new breast mound after a mastectomy. The advantage of this type of surgery is that the reconstructed breast is very soft, natural and lifelike. The TRAM flap reconstruction has been performed on women ranging in age from their late twenties to their mid-seventies. Dr. Bidic has extensive experience with the TRAM flap procedure and has been performing this procedure throughout his career.
DIEP Flaps (deep inferior epigastric perforator)
In DIEP flap reconstruction, only skin, fat, and blood vessels are removed from the lower belly without removing any muscle. (The TRAM flap procedure removes muscle along with fat, skin and blood.) Because no muscle is removed, most women recover more quickly from DIEP compared to TRAM and have a lower risk of losing abdominal muscle strength. There also tends to be less abdominal wall discomfort because the muscle isn't involved. Like the TRAM flap, the patient will end up with a tummy tuck as a fringe benefit of DIEP surgery because fat from the abdomen is removed to reconstruct the breast and loose skin is tightened up. Breast reconstruction using the patient's own tissue is becoming more popular because it is a long-lasting solution and the consistency of the belly tissue is very similar to natural breast tissue. However, the new breast will have little, or no, sensation.
Tissue Expander Based Reconstructions
In many instances, post mastectomy breast reconstruction often requires the use of tissue expanders to help create room for the soon to be placed implant. The reason for the interim expanders is that following mastectomy surgery not only is less skin remaining at the breast site, but also the skin that does remain does not have a sufficient blood supply to accommodate the stretch that a final implant would place on the breast skin. In order to accommodate the new implant, the skin at the site of the breast must therefore be slowly stretched in order for new growth to occur, and the final implant to be comfortably placed in position. To accomplish this, a teardrop-shaped, balloon-like tissue expander must be surgically inserted under the chest muscle. Then over the course of the following weeks, saline solution is injected into the tissue expander. Once the tissue expander has sufficiently molded the skin, it is replaced with the permanent implant. Tissue expanders are also often used immediately after a mastectomy to maintain the breast pocket and expand the breast skin prior to radiation therapy in preparation for a future breast reconstruction option that utilizes the patients own fat and skin. Dr. Bidic has extensive experience with tissue expander reconstructions and has been performing this procedure throughout his career.
TUG Flaps (transverse upper gracilis flaps) from Medial Thigh
Similar to SIEA flap and TRAM flap reconstructive surgery, the TUG flap procedure differs in that tissue is taken from the inner thigh of the patient rather than the abdomen. There are two important benefits of this. First the scar from the upper, inner thigh donor site is easily concealed and infrequently viewed. The other advantage to this procedure is that there is no abdominal wall incision thereby lessening the recovery time for the patient. This is an excellent option for women desire using their own tissue to create a mew breast mound, but have had abdominal surgeries in the past that preclude the use of the abdominal-based flaps. Dr. Bidic has extensive experience with the TUG flap or Gracilis flap procedure and has been performing this procedure throughout the course of his career.
Latissimus Dorsi Flaps
The Latissimus Dorsi breast reconstruction utilizes skin, fat and muscle from the area just superior to the bra-strap line in the patient's back to create a new breast mound. This flap can be used alone or with implants. The Latissimus Dorsi flap can also be molded with tissue expanders to create a preferred optimal size and shape. The Latisimus Dorsi flap breast reconstruction is a versatile option providing outstanding results for many patients requiring breast reconstruction regardless of other complicating factors including radiation, tobacco use and obesity.
